This historical study shows how San Francisco and Baltimore were central to American expansion through the eighteenth and nineteenth centuries.   The history of the United States is often told as a movement westward, beginning at the Atlantic coast and following farmers across the continent. But early settlements and towns sprung up along the Pacific as well as the Atlantic, as Spaniards and Englishmen took Indian land and converted it into private property. In this ambitious study of historical geography and urban development, Mary P. Ryan reframes the story of American expansion.   Baltimore and San Francisco share common roots as early coastal trading centers immersed in the international circulation of goods and ideas. Ryan traces their beginnings back to the first human habitation of each area, showing how the juggernaut toward capitalism and nation-building could not commence until Europeans had taken the land for city building. She then recounts how Mexican ayuntamientos and Anglo-American city councils pioneered a prescient form of municipal sovereignty that served as both a crucible for democracy and a handmaid of capitalism.   Moving into the nineteenth century, Ryan shows how the citizens of Baltimore and San Francisco molded the shape of the modern city: the gridded downtown, rudimentary streetcar suburbs, and outlying great parks. This history culminates in the era of the Civil War when the economic engines of cities helped forge the East and the West into one nation.

In Modernizing Medicare, editors Robert Emmet Moffit and Marie Fishpaw bring together a rare combination of leading scholars and policy practitioners to outline a vision for Medicare reform and provide solutions for the millions of seniors whose health care depends on it. Contributors include a former Medicare trustee, a former Medicare administrator, and a former director of the Congressional Budget Office.Detailing Medicare’s biggest problems, this team of top policy experts offers solutions based on personal freedom of choice, transparency of price and performance, and market competition among health plans and providers that will secure patients more affordable, more accountable, and higher quality medical care. They also address Medicare’s reform needs and analyze the promising performance of the Medicare Advantage program. Contributors outline Medicare’s major financial problems and the best solutions for Medicare patients and taxpayers alike. While Medicare’s accelerating spending is generating higher deficits and debt, standard cost-control strategies—such as payment reductions and price controls—jeopardize patients’ access to high-quality care.

Scientists call this the Golden Age of the Common Cold because Americans suffer up to a billion colds each year, resulting in 40 million days of missed work and school and 100 million doctor visits. They've also learned over the past decade much more about what cold viruses are, what they do to the human body, and how symptoms can be addressed. In this ode to the odious cold, Jennifer Ackerman sifts through the chatter about treatments-what works, what doesn't, and what can't hurt. She dispels myths, such as susceptibility to colds reflects a weakened immune system. And she tracks current research, including work at the University of Virginia at Charlottesville, a world-renowned center of cold research studies, where the search for a cure continues.

Conventional wisdom has North America entering a new era of energy abundance thanks to shale gas. But has industry been honest? Cold, Hungry and in the Dark argues that declining productivity combined with increasing demand will trigger a crisis that will cause prices to skyrocket, damage the economy, and have a profound impact on the lives of nearly every North American.Relying on faulty science, bought-and-paid-for-white papers masquerading as independent research and "industry consultants," the "shale promoters" have vastly overstated the viable supply of shale gas resources for their own financial gain. This startling exposé, written by an industry insider, suggests that the stakes involved in the Enron scandal might seem like lunch money in comparison to the bursting of the natural gas bubble. Exhaustively researched and rigorously documented, Cold, Hungry and in the Dark:Puts supply-and-demand trends under a microscopeProvides overwhelming evidence of the absurdity of the one hundred-year supply mythSuggests numerous ways to mitigate the upcoming natural gas price spikeThe mainstream media has told us that natural gas will be cheap and plentiful for decades, when nothing could be
