In today’s complex healthcare environment, implementing evidence-based care into real-world practices is difficult and time consuming. Even methods that are known to be effective allow for limited flexibility and therefore fail as often as they succeed. Through much study and experimentation, Malaz Boustani, MD, MPH, Jose Azar, MD, and Craig A. Solid, PhD have come to understand how individuals’ interactions within the complex social systems of hospitals, clinics, and other care delivery organizations shape the decisions and behaviors of those involved. Upon this foundation and through leveraging theories of behavioral economics, we have developed the Agile Implementation Model, a process for selecting, adapting, implementing, evaluating, sustaining, and scaling evidence-based healthcare interventions. 
 
This model acknowledges the uniqueness of each individual facility and considers individuals within the system to be semiautonomous but interconnected. In tandem with illustrative examples, Agile Implementation describes the underlying theories and frameworks that explain health delivery systems and lays out the 8 steps of the Agile Implementation Model. Upon completing Agile Implementation, readers have a better understanding of why certain quality initiatives succeed while others fail and have tangible, actionable tools for implementing effective and sustainable change in the healthcare setting.

    This case report describes the poor outcome of a female patient who underwent elective nephrectomy. Unfortunately, the patient was suffering from undiagnosed preoperative platelet dysfunction and anemia, although blood count and clotting were almost within normal ranges two weeks earlier at the day of preoperative assessment for anesthesia. In this respect, intensive preoperative evaluation of the coagulation system embedded in a comprehensive patient blood management (PBM) concept might have the potential to detect preoperative coagulopathy and anemia and thereby could improve clinical outcome.

    Patient Blood Management (PBM) is a multidisciplinary, evidence-based approach to improve perioperative patient care. The concept includes preoperative screening, diagnosis and treatment of anemia and hemostasis, adequate blood product utilization and blood-sparing techniques.     Mike Owen, born with two congenital heart defects, battled with cardiac issues until the age of seven, when he underwent open heart surgery that changed his life. 
     
    For the following 31 years, Mike was plagued with intense and disabling heart palpitations. In 2004, the cause of the palpitations was diagnosed and, after a short cardiac procedure, for the first time in 38 years his heart was stable. 
     
    Life became easier. His confidence grew. But so did his workload. After three years of work-related stress, his heart succumbed to a new type of palpitation which would blight his life for the next four years. After taking steps to reduce the frequency and intensity of his cardiac problems, Mike finally felt like he had turned a corner. However, in 2017, he was plunged back into despair following a series of dramatic events.  
     
    When his Occupational Health Doctor gave him an unexpected Mental Illness diagnosis, Mike embarked on a new path. He now had to deal with psychological issues, learn mindfulness techniques and work through the trauma of his childhood and the uncertainty of having spent so many years living with his heart problems. 
     
    With his faithful dog, Coco, always by his side, Mike confronted the stigma and discrimination surrounding mental illness, in his own way. He found physical, emotional, and mental stability thanks to his diagnosis, and this is his uplifting story.
